In-game instructions; substituting substitutes unintentionally

Even though I've arranged the players and made sure that I'm not taking anyone off on 75 minutes that has been brought on in 60 minutes, this still sometimes happens depending if the in-game instruction takes place. Is it perhaps to do with the order of it? Do I select formation, swap the players around, then make my substitute? Cheers
