Soccer Manager believes that most of the answers to your questions are covered within our Help.

Its possible that the manager may have quit the game world after receiving the offer. Obviously the offer can't be withdrawn. It will only expire after 5 days if not responded to. I'm not sure about this but I think I read somewhere that it is possible to offer jobs to external game world managers in some leagues based on their settings. So that maybe a reason.
