Soccer Manager believes that most of the answers to your questions are covered within our Help.

This would depend on how many games you've played them in, and also their rating. For example, if a rating 90 has a concern, then you may have to play him in 80% to 90% of your games for his concern to go down. If a rating 85 had a concern, then he would only have to play 4 or 5 games for his concern to go down. My guess is that you need to play him a bit more.
