Soccer Manager believes that most of the answers to your questions are covered within our Help.

SM labels teams under the country of the league that they play in, not where they are based. For example, Toronto FC plays in the MLS and their country is listed as United States even though they are from Canada. The same goes for San Marino who play in Italy, but are from the country of San Marino.

In any case, it is not an error, it is just how SM chooses to label the teams.
