Soccer Manager believes that most of the answers to your questions are covered within our Help.

Any offer you make to an EXTERNAL club should be accepted unless the values of the players concerned change. This is unusual. Try again, and if it fails submit a Bug Ticket.

I assume you are not talking about UNMANAGED clubs which are subject to different rules.
