How are players rated ... really?

Can anyone expand on the rating of players? I can't understand how some of the ratings are reached. For example Mariano Andujar of Catania. Since moving from the Argentine Premier to Italy and playing fully for 2 seasons he has never had his rating increased (also played for Argentine national squad). His rating has just dropped by one for no apparent reason, he hasn't missed a game or been relegated. The reason I ask is according to the guidelines, this would be a player who should have his rating increased , not decreased. Thanks
