Even though I've arranged the players and made sure that I'm not taking anyone off on 75 minutes that has been brought on in 60 minutes, this still sometimes happens depending if the in-game instruction takes place.
Is it perhaps to do with the order of it? Do I select formation, swap the players around, then make my substitute?

Hard to say based on what you have said...
Say you set instructions at 60 minutes to bring on (for example) Messi in position 9 for David Villa. If you have also set an instruction for the 75th minute to bring off Villa for Torres, then if the 60th minute instruction took effect then obviously Villa is no longer on the pitch so you would be taking off Messi.
Keep it simple and avoid making multiple changes that involve subs. I tend to just make tactical changes primarily.
