How can I make new formation?
you can find the best formation for your squad by arranging friendlies to test out differnet ones. but i like using 4-4-1-1 with wingers with forward arrows.

It all depends on the type of players you have and their ratings.
If u have good defenders go for 5-3-2 or 5-3-2Defensive.
For midfield go for 3-5-2 or 3-2-2-2-1.
For attack 3-4-2-1 or 3-4-1-2
For balanced formations 4-2-3-1, 4-4-2 and 4-3-3 are best.
Other formations are yours for experiments.
Remember it all depends on the players and also depends on your mentality.
And also u cannot make a new formation. It is on the SM suggestion so it could be implemented in the future.

For me, the best is: 93-75-4
That way, you crowd your defense and midfield not allowing anyone to score, plus, you have 4 strikers to put the ball in the back of the net... It works with me...

Its up to you, the quality of your squad and the opponents club formation, quality,etc. So, there arent any standards.
