I manage almost 40 clubs in different GWs and I recently noticed that one player's contract was about to expire (playing last game of the season). What would have happened if I'd missed it?
New Contract
Towards the end of the season you will be notified within your Club Messages of any players that are in the final year of their contract and you can approach them to offer them a new contract for either 1, 2, 3, 4 or 5 years. Likewise if you are desperate to tie down a player, it might be wise to offer them a new contract.
Prior to offering a new contract you will be notified the wage the player expects up to, and their agent and your Chairman will negotiate the exact amount.
If you have just signed a new player or you have just offered a new contract to a player, you will not be able to give them a new contract until the next season. The only exception to this rule is if your player has a concern (see Player Concerns) over their wage, then you will be able to offer them a new contract.

You will receive a warning but to be honest nothing happens even if the contract does expire.
The contract simply renews. The salary may increase or reduce depending on whether the player's rating has increased/decreased since he was last given a contract.
Generally, wages have gone up since SM started so wages will go up even if rating has remained the same. I.e. a 90 rated player demands a much higher wage then he did when SM began.

Yes, you will receive a message in your inbox at the end of the season in which the player's contract will expire.
